Clinic 35 - a partner in your child’s health care.
Welcome to Clinic 35! We are a School Based Health Center (SBHC) located inside of East Haven High School. We offer students a range of primary care services, with the convenience of being right at school and at no cost to the student or family. From sick visits to sports physicals, our on-site care team can partner with your child’s regular provider or become their medical home.

Clinic 35 is part of Fair Haven Community Health Care, a greater New Haven area health leader for 50 years. Clinic 35 is led by a Nurse Practitioner, Mary Patricia Lamberti, DNP, APRN, ANP-BC. Mary Pat has been the face of Clinic 35 since its opening in November 2017.
Receiving care at Clinic 35 will not replace your child’s regular health care clinician but supplements your child’s health care team. We work with the school nurse and your child’s other providers to ensure the best possible care to your child. We take a developmental approach to adolescent health, respecting the age, cultural values, and family life of every student.
Health Care Services
Clinic 35 offers free rapid COVID testing for students who are symptomatic. Students can drop by the clinic to ask about receiving a free rapid test.
We partner in your child’s health, by providing:
- Well-child physical examinations
- Sports physicals
- Sexual health education & services
- Diagnosis & treatment for injuries & illnesses
- Urgent care
- Immunizations & laboratory tests (Metabolic panels, rapid strep & flu tests)
- Annual Flu shots
- Management of chronic health conditions
- Prescriptions & over-the-counter medications
- Medication dispensing
- Nutritional health counseling
- Referrals for behavioral health counseling (in school with CHC, Inc.)
- Referrals for specialty care, including dental
- Referrals for addiction medicine services, including vaping, marijuana, and alcohol use
- Individual, group & classroom health education

Students are enrolled as patients of Fair Haven Community Health Care (FHCHC) and can seek care at any of our other locations in the greater New Haven area.

No hidden costs or fees
There is no cost to students or their families to be a patient at Clinic 35. For students with private insurance or HUKSY (Medicaid), we will bill the insurance company directly for eligible services. No co-pay is ever required.
Easy Enrollment
Parents or guardians must complete and sign a permission form to enroll each child under 18 years of age. Students 18 or older can enroll and sign permission forms for themselves.

Appointments when your child needs it
Students or their parents/guardians can make an appointment at Clinic 35 by calling 203-467-2765, during normal hours. We are open Monday to Friday from 7:00 am to 3 pm. For urgent care, no appointment is required. Students can drop in anytime during clinic hours, as long as they have a pass from their teacher.
COVID Vaccines
Individuals ages 12 and older are now eligible to receive a COVID vaccine. Students and families that are interested in being vaccinated should contact Clinic 35. Fair Haven Community Health Care is also offering COVID vaccine appointments. Please call 203-871-4179 to schedule an appointment.
Flu Shots
Clinic 35 offers flu shots for all patients, at no charge.

East Haven High School is happy to announce a partnership with Community Health Center, Inc. who has opened a School Based Health Center (SBHC) in our building. Attached please find a flyer which describes the services that the SBHC will be providing to our school community, as well as a ‘Frequently Asked Questions’ document. Licensed healthcare providers are available to provide expanded medical (treatment for illnesses or injuries, and physicals), behavioral health (individual, group, and family therapy), and dental hygiene (exams, cleanings, and sealants) services. School-based health services work in conjunction with the care provided by your child’s pediatrician and/or dentist and are not intended to replace regular care by your child’s primary health-care provider. Please note that parents/guardians must enroll their student in the program in order for the child to be seen.
